New Patients

    We welcome new patients to our practice. When you come to our office, a new patient information packet will be given to you. This will include a registration form for you to fill out. Please bring your current health insurance card along with any prior records or labs we may need for your appointment. If you have been referred to us by another physician, please make sure that we have your referral. You can ask their office to fax to us prior to your appointment.


    Dr. Kundu sees patients in the office on Tuesday and Thursdays. For urgent appointments, we will try to accommodate you the same day if possible. For routine and annual appointments, it may take approximately two weeks. If you are scheduling an appointment for an annual examination, please make sure it has been over a year sine your last visit.

Phone Calls

    We welcome and encourage you to call our office if you have any medical problem or questions. For complicated questions, which may take time for explanation, please make an appointment.

Medical Refills

    If you need a refill on your medication, please call your pharmacy and have them fax us the request. If you need a new prescription or are out of refills, please call our office and talk to one of our staff. We will call the pharmacy for you. Please give us two to three working days for routine refills.

Payments / Fees

    Our fees are within the customary range for this area and reflect the level of care you will receive. It is our intent to keep our charges as reasonable as possible. We have found that the billing procedures add unnecessary expense to your care. Therefore, fees for office visits and tests done in the office are to be paid at the time of the visit. We recommend that you discuss fees for Gynecologic surgery with our office manager to arrange payment schedules that will be convenient acceptable.


    You are responsible for determining whether a service is covered by your health insurance. Please make sure you give us your correct health insurance card so that we may send you to the correct participating lab under your health insurance.


    Dr. Kundu is affiliated with Sacred Heart & Baptist hospital.

Medical Records and Confidentiality

    All of your medical records are confidential and we will not release the records of any part of it unless you give us the written authorization. No matter what the circumstances, we will not release records unless we have notification from you.

Laboratory Services

    Please check with your health insurance company prior to your visit to determine which laboratory your samples should be sent to. We recommend that you set up a follow-up appointment to discuss your test results unless Dr. Kundu advises otherwise.


    The receipt that is given to you after your visit has been designed to be used for insurance purposes. Our office staff will file insurance claims for Medical and Surgical care. We will help you in whatever way we can, but you can help us save time and serve you better if you make sure that your particular insurance policies cover the services for which you want to file claims. Your insurance is generally a contact between you and your insurance company. You are responsible for your bills regardless of what your insurance company pays.
